Please note this service is commissioned across Cheshire and Merseyside by the British Heart Foundation (BHF). Therefore, the following LPCs have the same service in their areas:
- Cheshire & Wirral LPC;
- Halton, St Helens & Knowsley LPC;
- Liverpool LPC; and
- Sefton LPC.
This service is Round 2 of a British Heart Foundation pilot which focussed on detecting undiagnosed hypertension. Contractors must have taken part in the BHF Round 1 pilot in order to be eligible to provide this service.
The service is structured as follows:
- Registration
- First blood pressure measurement
- Second blood pressure measurement
- Follow-up call three months after the second blood pressure measurement.
