Description
The Transfer of Care Around Medicines (TCAM) scheme facilitates the accurate transfer of a copy of a patient’s dispensed discharge prescription from the hospital pharmacy to the patient’s chosen community pharmacy subject to patient consent.
This project is endorsed by Barnsley LPC, Barnsley Hospital NHSFT, Barnsley CCG and supported by the Yorkshire and Humber Academic Health Science Network.
Research shows that medicines-related problems after discharge from hospital can be addressed by more systematic involvement of community pharmacists.
Patients discharged from Barnsley Hospital NHSFT will have their medication information sent to the community pharmacy of their choice upon discharge unless that pharmacy has opted out of the process. Information will be sent via Pharmoutcomes.
Community Pharmacy once alerted of the discharge can continue to provide pharmaceutical care in the community offering a range of services including a Medicines Use Review (MUR) and or a New Medicines Service (NMS).
