Services Database Atrial Fibrillation Service (Trafford Area)

Greater Manchester LPC is working collaboratively with the Greater Manchester Academic Health Science Network on an innovative project to reduce the stroke risk of known atrial fibrillation (AF) patients within primary care. The community pharmacists involved will work jointly with GPs to manage AF patients by undertaking an in-depth anticoagulant consultation with the AF patient.

The aims of this service are to:

• reduce stroke prevalence and complications;
• increase patient engagement and compliance with AF therapy;
• increase the number of patients with AF being prescribed anticoagulant therapy;
• ensure that individuals diagnosed with AF have access to information about their condition and treatment options alongside lifestyle advice;
• improve community pharmacists' role in care and improve GP implementation of NICE guidance; and
• ensure a patient-centred approach to care.

• Prior to provision of the service the pharmacist must complete face to face training, completed the CPPE packages “Consultation skills for Pharmacy practice” and “Anticoagulants” with associated assessments, documented learning of the “Prescriber Decision Support of New Oral Anticoagulants by GMCSU December 2013” and have reviewed the service specification, service guide and signed the Service Level Agreement.
• Face to face training will be provided by the LPC with support from CPPE.
