Service Aim
- To establish a proposed model of care for future commissioning of NHS community
pharmacy clinical services incorporating cholesterol POCT to identify people at risk
of CVD enabling primary prevention strategies.
Service Objectives
- Community pharmacy team to undertake cholesterol POCT.
- To identify local residents that are at risk of CVD, using the lipid results from the
POCT, in line with national guidance and help address the local health inequality. - Offer lifestyle advice to all people being screened and refer high risk people to their
GP for lipid lowering therapy management for primary prevention of CVD.
