Please note this service was previously called Medication Management System.

This service aims to provide the service user and/or their carer with support in relation to medication which has been prescribed. Pharmacists participating in the service will undertake a Medicines Use Review (MUR), from which they produce a Medication Plan, detailing appropriate support to meet the specific needs of the service user. Whether the service user suffers from cognitive decline or degenerative disease, the service aims to enable them to remain in their own home.

The core elements of the service will be the provision of appropriate compliance aids by participating pharmacists, this may include the provision of Monitored Dosage Systems (MDS) complimented by the use of Medication Administration Records (MAR) for recording purposes.

Referrals are made by community / hospital teams once a service user has been identified as unable to self-medicate. Once a referral has been made the pharmacist in receipt of the referral will contact the service user and/or next of kin to make an appointment to undertake an MUR and begin the initial process of the service. The service is usually delivered in the patient’s home.

Training

Participating pharmacists should have the appropriate DBS check for working with vulnerable adults.
All contributing parties undergo a training plan which is refreshed every three years.
