Description
Pharmacists and their teams identify prescription interventions and communicate this information to the GP practices using a designated form (usually via the practice medicine manager). The interventions are processed by the GP practice and the community pharmacy receives feedback on the result.
The types interventions are listed in the scheme details and are designed to identify cost saving, waste reduction and quality medicine management issues over and above those traditionally highlighted during the dispensing process.
Payments are made based upon the interventions identified and potential savings generated.
