Description
The aims of the HOMe Service are to optimise housebound patients’ medicines to maximise the value that patients derive from their medicines, to reduce hospital admissions and/or readmissions and to reduce medicines waste by:
- improving the patient’s understanding of their medicines and why they are taking them;
- improving access to medicines by putting in place systems for the ordering and/or collection of prescriptions and/or delivery of medicines where needs are identified;
- improving adherence to medication by putting in place systems to support adherence where needs are identified;
- using the expertise and knowledge of pharmacists on the use of medicines to reduce the incidence of adverse drug effects and/or side effects where possible;
- checking that appropriate monitoring of medicines is occurring;
- reducing wastage of medicines;
- arranging for carer disposal of medicines no longer required and/or are out of date;
- producing measurable outcomes on the likely reductions in emergency hospital admissions as a result of the interventions made; and
- informing future developments in pharmaceutical services to the housebound population.

Training
The following requirements need to be met:
- Ensure named pharmacists delivering the service have a current DBS check (within the last 2 years);
- Declare that the pharmacy is IG level 2 compliant; and
- Ensure named pharmacists delivering the service have attended any relevant training sessions by Kingston CCG.
