Description
The Leeds Blood Pressure Wise Service is commissioned by Leeds City Council in six pharmacies across Leeds. The service will fund a total of 200 blood pressure checks per year over a two-year period for each commissioned pharmacy (a total of 2,400 blood pressure checks).
Leeds Blood Pressure Wise Service has been commissioned to increase the identification of hypertension and atrial fibrillation by offering opportunistic blood pressure screening in community pharmacies. Aims of the service include:
- to increase the detection and management of hypertension and atrial fibrillation;
- to provide easy access to blood pressure monitoring through community pharmacies;
- to develop and pilot a new method of data collection and data sharing using a SystmOne Community Module; and
- to encourage self-management and healthy lifestyles to reduce risks of developing hypertension.
The service will be offered to any person over the age of 35 years old that meets the criteria outlined in annex 3 of the service specification (attached).

Training
- The health care practitioner must have completed Leeds Blood Pressure Wise training delivered by Leeds City Council prior to the delivery of the service.
