Services Database MAR Chart Provision (South Tyneside Area)

This enhanced service seeks to ensure that patients who need additional help with taking their medicines are supported safely by their Social Care & Health / Privater Agency carer with the preparation of a Medicines Administration Record chart by their community pharmacy.

Pharmacists will provide this service to the patients whose carer (whether employed by Social Care & Health or a Private Agency) needs to make a record of medicine administration to, medicine prompting to, or medicine refusal by the patient.

This enhanced service only covers patients who need carer support with the medication. Patients who need additional support to manage their medication without carer support should be assessed under the DDA legislation and the appropriate support provided. Clients may self-refer or their carer may request the service on their behalf. Anyone requesting the service must sign a completed request form, to be kept in the pharmacy for inspection if requested.
