Services Database Provision of Treatment Meticillin Resistant Staphylococcus Aureus (MRSA) Service (Wakefield Area)

The purpose of this service is for community pharmacies to provide:

• a service for and supply Meticillin Resistant Staphylococcus Aureus (MRSA) suppression treatment to patients testing positive following MRSA screening using advice from the consultant microbiologist at Mid Yorkshire Hospitals NHS Trust; and
• advice in the form of a consultation to ensure patients are aware of how to administer the treatment.

The patient and the patient's GP practice will receive a letter that will include information on the form of MRSA resistance. Decisions regarding whether the patient should receive dual or triple therapy and which drugs are appropriate will be clarified for the community pharmacist within the patient's referring letter.

The supply of dual therapy is in the form of:

• Bactroban (Mupirocin) 2% nasal ointment, 3g tube; and
• Chlorhexidine (Hibiscrub) Solution body wash lotion, 500ml.

Patients that are resistant to the first line of treatment i.e. Mupirocin will be supplied with either Naseptin or Prontoderm, dependent on the resistant pattern. This information will be detailed in the letter to the patient to be presented to the community pharmacist for the supply of treatment.

Only when stipulated for patients having a percutaneous endoscopic gastrostomy (PEG) feeding tube inserted and who have been screened as MRSA positive will triple therapy be supplied:

• Bactroban ( Mupirocin) 2% nasal ointment, 3g tube;
• Chlorhexidine (Hibiscrub) Solution body wash lotion, 500ml; and
• Chlorhexidine gluconate 0.2% mouthwash, 300ml.

Where patients are unable to apply the five day suppression treatment the community pharmacist will refer the patient via the single point of access to the District Nursing Service.

Please note, this service has now been decommissioned.

The pharmacy contractor will have a duty to ensure that pharmacists involved in the provision of the service are aware of and operate within local protocols.

Pharmacists who supply and administer medicines under the suite of PGDs must undertake any local training that the commissioner may require them to.
