Description
The Fleetwood Neighbourhood of GPs has chosen to commission a pilot pharmacy minor ailment scheme from community pharmacies across the Fleetwood Neighbourhood (part or Fylde & Wyre CCG) for the 30,000 patients registered to the four GP practices included in the Prime Ministers Challenge Fund (PMCF) pilot.
Patients can be referred into the service by:
- a participating GP - practice or other healthcare professional patients presenting at a participating GP surgery, out of hours, or same day service with one of the minor ailments listed in Schedule 5 may be advised of the scheme and how to access it through their local pharmacy. Surgeries etc should note that patients who are not exempt from prescription charges will be required to pay for any medication supplied.
- the patient self-referring into the service - who are aware of the scheme, whether through friends, relatives or a healthcare professional can self-refer into the service.
- a pharmacy team member - pharmacists and pharmacy staff may refer patients into the scheme where they feel it is appropriate.
