Description
The service is a management of minor ailments provided by community pharmacists within Sutton.
The service is a fast track referral service that allows patients:
- to be referred from General Practice or a Walk-in Centre to a community pharmacy for appropriate treatment;
- self-referral by Sutton & Merton CCG registered patients to a Sutton & Merton CCG Community pharmacy for suitable treatment; and
- referral from drug & Alcohol team, GP out of hours services, opticians and dentists and NHS 111.
Community pharmacists will take part in a patient survey of the service. Community pharmacists will be expected to provide at least 25 survey forms.
