Description
The pharmacy will dispense medication directly to a patient who presents with a signed order on the agreed form written by a registered ophthalmic practitioner participating in the Minor Eye Conditions Service. The patient will be registered with a Hackney GP.
Only specific products listed in the Service Level Agreement may be supplied under this scheme.
