Description
The Self-Management Support Programme is a local service in Newham provided by community pharmacists, which helps and supports patients with long term conditions to consider the activities, lifestyle and behaviour that they want to adopt to manage their long term condition(s) better and identify local services which can support the activities and changes identified.
People taking part will be supported by a trained pharmacist over a 12 week period through one-to-one coaching sessions to develop a personalised action plan, known as a wellbeing plan that will identify lifestyle changes that will help to improve health and wellbeing.
The service is available to all eligible patients who are registered with a General Practitioner (GP) in Newham who have 2 or more of the following long term conditions: atrial fibrillation, coronary heart disease, cardiovascular disease, chronic obstructive pulmonary disease (COPD); asthma; stable hypertension; or controlled diabetes. All referrals must come from a GP.
This service consists of three sessions with a trained pharmacist over a 12-week period where they will support patients to identify the changes that will help them better manage their long-term conditions by developing a wellbeing plan.
