Description

The NHS Health Check Programme is a public health programme for people aged 40-74 which aims to keep people well for longer. The NHS Health Check should be offered to all eligible people once every five years. It is a risk assessment and management programme to prevent or delay the onset of diabetes, heart and kidney disease and stroke. The programme also aims to reduce levels of alcohol related harm and to raise awareness of the signs of dementia and where people can go for help.

Pharmacies will use the Swindon CVD computer system which has been designed to ensure that the health check is consistently delivered throughout the Swindon area and that the patient has not already had a health check in a community venue.
