Description
The service requires the pharmacy team to fill palliative care boxes with emergency medicines, which may be needed in the future. The nursing team work with the GPs to identify patients and then write relevant prescriptions which are filled by the pharmacy and packed into a palliative care box, which are then stored in the patient’s home.
This service can only be delivered in areas where the district nurses and the GPs are happy to support it, therefore it may not be offered in every pharmacy in the area.
This service has been decommissioned because there were a number of issues with stock availability for some palliative care medications. Of particular concern are the issues surrounding haloperidol 5mg/mL injection, some strengths of diamorphine and now cyclizine injection.
