The target cohort is those aged 16-40, who are dispensed medicine for diabetes (Type 1 or Type 2).

The project aims to reach 700 patients within the cohort.

Each participating pharmacy will be asked to:

  • search Patient Medication Records;
    • identify patients in the cohort;
    • highlight the record to facilitate intervention at next dispensing;
  • engage with each identified patient as they collect medicines;
    • discuss awareness of long term complications;
    • give out and discuss blue book;
    • reinforce and emphasise the importance of the care processes and the need for them to be carried out annually;
    • signpost to appropriate online materials;
    • invite for Medicines Use Review if discussion indicates this is appropriate;
  • follow-up in 1-3 months;
    • identify if any of the resources have been helpful.
