Services Database Promoting Self Care in Young People with Diabetes – Pilot (Wiltshire Area)

Description
The target cohort is those aged 16-40, who are dispensed medicine for diabetes (Type 1 or Type 2).
The project aims to reach 700 patients within the cohort.
Each participating pharmacy will be asked to:
- search Patient Medication Records;
- identify patients in the cohort;
- highlight the record to facilitate intervention at next dispensing;
- engage with each identified patient as they collect medicines;
- discuss awareness of long term complications;
- give out and discuss blue book;
- reinforce and emphasise the importance of the care processes and the need for them to be carried out annually;
- signpost to appropriate online materials;
- invite for Medicines Use Review if discussion indicates this is appropriate;
- follow-up in 1-3 months;
- identify if any of the resources have been helpful.
Timescales for this project are as follows:
- June 2015: invitation to participate
- July – September 2015: Pharmacies identify patients
- August 2015 – January 2016: Patient interventions
- September 2015 – February 2016: Patient Follow-up/feedback
- October 2015: Preliminary report to Wiltshire Council
- March 2016: Final report to Wiltshire Council
