Services Database Supply of Urgent Care Medicines by Community Pharmacy (Kent and Medway Area)

Description

  • Maintain an agreed stock of palliative and/or urgent care medications across Kent and Medway during and out of working hours at designated community pharmacies, intended for supply against an FP10 prescription.
  • Improve access to palliative care and other urgently required medicines in Kent and Medway for patients, relatives, and clinicians, through community pharmacies during normal working hours and extended hours.
  • The service will be delivered in Two Tiers, See below for specifics:

 

Tier 1-

  • These Pharmacies will supply medication on the EXTENDED FORMULARY LIST (Appendix 1)

This will include End of Life/ Palliative Care medicines as well as additional urgent medication on the formulary for treating C-Difficile within the community. This includes Octreotide, Fidaxomicin and Vancomycin.

  • They will have extended opening times to support access to EOL/PC and Urgent medicines.
  • Prescriptions will be dispensed within a reasonable timeframe (given the nature of medication).
  • The community pharmacy will contactable, and will provide up to date contacts details that will be shared with all relevant K&M health care professionals/organisations.
  • They will maintain stock levels of all medication on the service specification formulary and any stock shortages are highlighted during the monthly (and if stepped up weekly) calls.
  • Support people, relatives, carers, and clinicians by providing them with up-to-date information, accessibility, and stock information.
  • They will offer a delivery service to support with access to medicines. Community Pharmacies will follow their own internal delivery SOPs, which should outline a process for the delivery of Control drugs.

 

Tier 2-

  • These pharmacies will ONLY Stock EOL/PC medicines.
  • They will be open for access to medicines within their normal hours.
  • Prescriptions will be dispensed within a reasonable timeframe (given the nature of medication).
  • The community pharmacy will contactable, and will provide up to date contacts details will be shared with all Relevant K&M health care professionals.
  • They will maintain stock levels of all medication on the service specification formulary and any stock shortages are highlighted during the monthly (and if stepped up weekly) calls.
  • Support people, relatives, carers, and clinicians by providing them with up-to-date information, accessibility, and stock information.

Training

  • The Community Pharmacy involved in the provision of this service should have relevant knowledge and understanding of End of Life and Palliative care as well as a good understanding of C- Diff.
  • The provider will ensure there is sufficient workforce in place across all the settings with knowledge to deliver this service and staff involved in providing any aspect of care under this scheme have the necessary training and skills to do so.
  • The provider (this term includes its employees, agents and all others authorised by it) shall respect the confidentiality of all information relating to patients, carers, or other users of the service.
  • They will comply with The Confidentiality and Disclosure of Information: General Medical Services, Personal Medical Services and Alternative Provider Medical Services Directions 2021, Caldicott, the Data Protection Act 2018 and all other relevant legislation.
  • The K&M ICB reserve the right to audit physical stock checks and unannounced visit for stock checks.
