Please complete the details below to report problems experienced in obtaining a product listed in Part IX of the Drug Tariff — whether it be extra charges being added, higher than Drug Tariff price, or difficulties in obtaining. Community Pharmacy England will work to ensure this information is fed into the Department of Health and Social Care as evidence of the ongoing problems with the supply of appliances.
