Description
Community pharmacies are commissioned to identify patients aged 40 to 74 years (30 to 74 years for South Asians) who are resident in Camden and are not currently being treated for CVD or monitored by their GP. They will screen them to establish their cardiovascular risk through completion of a questionnaire, measurements and finger-prick blood testing.
