Description
The pharmacy will identify pre-diabetic patients and invite them to participate in the National Diabetes Prevention Programme (NDPP). The pharmacy will be required to identify pre-diabetic patients based on a HbA1c of 42-47 mmol/mol or fasting blood glucose of 5.5-7 mmol/mol. Pre-diabetic patients identified should be invited to attend the NDPP, via PharmOutcomes.
