Description
The service aims to identify and manage people’s risk of a range of vascular diseases (i.e. heart disease, stroke, diabetes and kidney disease) and to offer them appropriate advice, referral or follow up.
All individuals receiving a Health Check should be given a Health Checks card advising them that they have had an NHS Health Check and informing them that their next Health Check is due in five years time. This is in order to help reduce the number of individuals attending for repeat Health Checks.
