Description
This project will enable patients to be referred from hospital to their community pharmacist for review of their medicines, which is especially important if they have started a new medicine or are having difficulties with an existing medicine.
Patients are assessed by a pharmacist at Dorset County Hospital and if they have started any new medicines, or are having difficulties or concerns about their medicines, they are, with their consent, referred directly to their community pharmacist for services such as a Medicines Use Review or the New Medicine Service.
