Services Database Weight Management Referral Service (Bristol Area)

The aim of this service is to improve motivation and support for Bristol residents who are overweight or obese to lose weight as part of a weight management action plan.

The service includes pharmacy teams:

• offering to weigh and take the height of Bristol residents so they can be provided with their body mass index (BMI);
• referring all clients with a BMI >50 back to their GP for referral into the Tier 3 service managed by the Clinical Commissioning Group; and
• providing data as required by the commissioner electronically via PharmOutcomes.

Priority areas include:

• Bristol residents living in areas described as average deprivation to most deprived (quintiles 3, 4 and 5);
• men;
• BME groups particularly South Asian, African Caribbean;
• long term chronic conditions including diabetes, hypertension, cardiovascular disease, arthritis/osteo-arthritis, metabolic syndrome; and
• mental health diagnosis or a learning disability.
