Description
The aim of this service is to improve motivation and support for Bristol residents who are overweight or obese to lose weight as part of a weight management action plan.
The service includes pharmacy teams:
- offering to weigh and take the height of Bristol residents so they can be provided with their body mass index (BMI);
- referring all clients with a BMI >50 back to their GP for referral into the Tier 3 service managed by the Clinical Commissioning Group; and
- providing data as required by the commissioner electronically via PharmOutcomes.
Priority areas include:
- Bristol residents living in areas described as average deprivation to most deprived (quintiles 3, 4 and 5);
- men;
- BME groups particularly South Asian, African Caribbean;
- long term chronic conditions including diabetes, hypertension, cardiovascular disease, arthritis/osteo-arthritis, metabolic syndrome; and
- mental health diagnosis or a learning disability.
